However, if you have to pay %50 of the premiums you may be better off with your own individual policy because an individual policy costs less (depending on what state you live in) and, you own the policy! With a group plan, when you leave your job you also leave your insurance.

Call youir local FBI field office and the State Insurance Commissioner, State Managed Care Director, Attorney General and the Governor's office and ask. They should be. Those health plans are nothing but rips offs. They cover less than they did 10 years ago and reimburse less than they did on surgeries 10- years ago.
I know, I just had the same foot surgery on one side as I did in 1996!
Blue Cross paid 4,216 in 1996 and 1475 for the other foot in Jan 2009!!!!
It depends if you are asking about the full time admin people, or the part timers that work in the retail stores. If you are asking about the retail clerks, I believe they have an Aetna SRC mini-med plan. Otherwise, it’s a Starbridge mini-med plan, which was recently bought by CIGNA. Premiums will run about $40 a month, but coverage is very limited.

Your best option is to call the phone number on your card. They can tell you based specifically on the plan your employer purchased and what requirements (if any) that you must meet (pre-certification or pre-authorization).
Hello, I have United Health Care PPO and they paid 100%. Call and talk with someone at United and Healthcare and see if the do approve it and if there is any deductible or co-pay. They should be able to tell you right away.
PPO's don't require referrals. Call them and ask who is contracted. There should be a phone number on the back of your card. Or you can call some psychiatrists and ask if they are contracted with your insurance company.
We have united health care insurance through my husband's job with the railroad (I do not know if that makes a difference?) It cost us $15. That included the delivery, epidural, hospital stay, the pediatrician to look at baby, all the newborn tests, everything. For my prenatal care was $15 also. So $30 total from first prenatal to bring baby home. What a blessing this insurance is!
Usually, employers do not administer urine exams on the day of the interview. If you are a viable candidate after the interview, they will schedule one for you (or ask you to schedule one at the lab of their choice). They do have the right but they must inform you first. You have the right to refuse the exam however, this could affect your chances of being hired.
I used to pay medical and dental claims for insurance companies so I can answer this with background knowledge, even though it's been awhile. Most all insurance companies pay for mental health care when the provider is a psychiatrist. However, if your preference is a counselor you usually have to get a referral from your family doctor or a psychiatrist for the insurance co to pay. If United has a list of providers that they want you to choose from, they will pay better benefits if you use one of them. If you don't choose a preferred provider with United, they will usually pay very little or nothing at all.
I would have to look at your benefit booklet to answer this question more directly. If you cannot find a provider on their website you can call United toll free and ask an agent if your provider is covered. I hope this isn't too confusing. Insurance is one of the hardest things to explain to people.

ppo is preffered medical plan, it means you can choose a doctor that you like or continue seeing a doctor that you are already seeing. epo is preselected md's that you have to choose on a list, that the company already picked, you're doctor you like may not be on there, that's why i picked the ppo, and no big deal its like $6 more. so get ppo if you want to choose a doctor you like. and trust me the doctors and specialists that the company chooses are real winners. meaning terrible

I think it's a typical large company. They use their imagination with their book-keeping. You're nothing but a number, BUT if you behave yourself, you'll make some $$. .
Call your insurance company and ask the customer service representative for a listing of drs (a provider directory) or where to go online for their site to find the listing of drs. Then when you find one call and ask about their wait time to get an appt. If it's for an annual exam it could be a few weeks, if it's because you have a problem such as an infection they will try to get you in sooner.

If he didn't enroll in this plan originally then I would say that the enrollment is fraudulent, but it can take a very long time to maneuver through the medicare red tape. Tell your sister to ask Rexall for the original sign up date and the supporting paperwork. She can also file a complaint about Rexall with medicare if the enrollment is fraudulent.
If, however, he did enroll at one time in the plan, then re-enrollment is automatic and he would have had to cancel the plan for 2007 by the end of 2006. This is common knowledge for the medicare part d program and your sister should have kept herself informed.
I helped my grandmother with her prescriptions and medicare part d plan, and they sent so much mail and email advising of the re-enrollment, it would have been hard to miss – the warnings of re-enrollment for non-action were very clear.

Yes. Secure Horizons is owned by United HealthCare. Secure Horizons offers mostly Medicare Fee-For-Service (FFS) plans. This means that physicians offices bill Secure Horizons INSTEAD of Medicare. Secure Horizons FFS acts, in a way, like Medicare plus a supplement. Secure Horizons then collects payment from Medicare. The monthly premiums to carry a FFS are way lower than a traditional supplement. But, your mom will have to pay a deductible, co-pays and co-insurance for services – whereas with a traditional supplement she would not. However, if she is in relatively good health, the FFS plan is definitely the way to go. I recently switched my grandmother from a traditional supplement to an Anthem FFS plan. FFS plans must provide at least the same coverage as traditional Medicare. The deductible, co-pays, and co-insurance amounts vary between FFS plans – the lower those amounts run, the higher the monthly premium. It's all about shopping around and finding the best balance. Yes, it's referred to as COBRA. Generally, you can extend your group coverage pursuant to COBRA for 18 months after you leave employment, provided you pay the full premium, plus an administrative fee (which your employer is entitled to charge).

Ask your vet whether or not your pet can travel "fasting". It would be best (dog-doo does stink and your cabin mates can't open a window) If yes, then all you'll need is to bring a bottle of either your own tap-water or buy some filtered water and keep her on that. Dog tummies are more sensitive to changes. Be sure to put a absorbent pad and fluffy towel on the bottom of carrier and a comforting toy (like a non-food chew bone).

Nothing is free. Universal health care would be paid for with higher taxes. Our current system is paid for with deductions from your pay check or you buy it privately like any other type of insurance. Either way you are paying for it.
